This Agreement is made between Young Muslims (YM) and the undersigned Participant (or Parent/Guardian if under 18). It applies to participation in Midwest Regional Tarbiyah event occurring January 1st-4th at Pokagon State Park, including related travel, lodging, and on-site programming.
1. Participation & Supervision
1.1 I understand that I am expected to remain within designated areas (the campsite and permitted areas) during scheduled programming and quiet hours.
1.2 If I need to leave the campsite, I will notify YM Senior leadership in advance, and I understand that I do so at my own risk.
1.3 YM reserves the right to restrict or remove participation for safety, disciplinary, or health reasons.
2. Travel, Transportation & Lodging
2.1 I am responsible for my transportation to and from the event. If I use YM-arranged buses or carpooling, I understand such transportation is voluntary and at my own risk.
2.2 YM may arrange lodging for participants. I will abide by all lodging rules and policies, and I understand YM is not responsible for personal property loss, theft, or damages in hotels or during unsupervised time.
2.3 Local attendees commuting from home are responsible for their own daily travel and punctuality.
3. Food, Purchases & Allergies
3.1 I understand YM is providing food for this event; and will communicate any necessary accommodations at least 3 weeks in advance of the Tarbiya Retreat.
3.2 I am responsible for managing my own dietary needs and allergies, verifying ingredients, and making safe choices. YM cannot guarantee allergen-free options and is not liable for reactions or food-borne illness from third-party vendors.
4. Safety & Conduct
4.1 I will follow all instructions from YM Senior Leadership and campsite managers and employees.
4.2 I will conduct myself respectfully and in a manner consistent with Islamic morals and YM values.
4.3 Reckless, inappropriate, or disruptive behavior, or leaving designated areas without notice, may result in removal from the event at my expense, without refund.
5. Assumption of Risk
5.1 I acknowledge that large public events involve inherent risks, including but not limited to crowds, slips and falls, illness, allergic reactions, travel incidents, theft, and interactions with the public.
5.2 I voluntarily assume all risks, known and unknown, arising from my participation in Midwest Regional Tarbiyah Retreat.
6. Medical Authorization
6.1 I affirm I am physically fit to participate and have disclosed relevant allergies, conditions, or medications to YM.
6.2 In an emergency, I authorize YM to provide or seek medical assistance and to contact my emergency contact as necessary.
6.3 I understand YM is not financially responsible for costs of emergency medical care, prescriptions, or treatment.
7. Waiver, Release & Indemnity
7.1 In consideration of being permitted to participate, I release, waive, and hold harmless YM, its officers, coordinators, directors, volunteers, affiliates from any and all liability, claims, demands, losses, or expenses arising out of or related to my participation.
8. Acknowledgment
8.1 I have read and understand this Agreement and sign it freely and voluntarily.
8.2 I understand that by signing, I am waiving certain legal rights, including the right to sue.
8.3 If the participant is under 18, I affirm I am the parent/legal guardian with authority to sign on the participant’s behalf and consent to these terms.
9. Drug Policy
I hereby approve that if I am seen with, or have possession of, any vape, illegal drug, alcohol, or any other form of medically un-prescribed substance, whether in my belongings or luggage, that I will be immediately expelled from the current retreat (WITHOUT REFUND), will need to arrange transportation myself in leaving, and I will no longer be invited to any future events. Furthermore, I hereby approve that I am subject to a search of my bag and personal belongings prior to and during the retreat. I understand that, if caught with any illegal substance, I will be reported to local authorities.
10. Disclaimer
For purposes of this Agreement, “I,” “me,” and “my” refer to the adult participant signing this Agreement, or, if the participant is under 18, to the parent/legal guardian signing on the minor participant’s behalf.
